Using an online form builder called Jotform, the presenter will share his experiences setting up dozens of music apps and forms and show you how to create your own in seconds USING VOICE PROMPTS!
The demonstration will include the Music Student App and Music Faculty App employed by MiraCosta College, forms for the Oceanside Jazz Festival, and the apps used for the California All-State Jazz Choir and one-day workshops put on by the Vocal Jazz Academy.

Matt Falker
Anchor Music Publications
Matt Falker is a jazz and gospel pianist, vocalist, arranger and clinician. Matt distributes his vocal jazz arrangements through his publishing company, Anchor Music Publications (formerly Sound Music Publications founded by Frank DeMiero in 1988), as well as the music of dozens of other vocal jazz arrangers and composers.
Matt is also the executive director of the Vocal Jazz Academy: a nonprofit organization dedicated to training and supporting vocal jazz educators primarily through a series of one-day jazz workshops for teachers and students. As a full-time piano and vocal jazz professor at MiraCosta College, Matt directs the Downbeat award-winning Frequency vocal jazz program, and hosts the Oceanside Jazz Festival. He also directed the Jazz Immersion Choir at the 2023 ACDA National Conference and served as a clinician for the 2026 Generations In Jazz Festival in Mount Gambier, South Australia.
